Important Information For Applicants
- Following the intake call with a Claims Services Representative, you will receive a package of forms bearing a file number. You will be asked to provide this number when contacting the Board, so that we can easily access your information. This number should also be written on anything you send to the Board.
- There will be pre-recorded information on your forms. These particulars were obtained during your initial contact with the Board. You must review this information to ensure it is correct. If your mailing address has changed, or if it changes at any point through the life of your claim, it is important that you notify the Board immediately. If there is an error on the forms, it can be corrected directly on the forms. Please note that all forms must be completed in ink, signed, and dated. Faxes, photocopies, and penciled documents are not accepted.
- Once a compensation analyst is assigned to your file, you will be asked to provide information in support of your claim. Where applicable, the Board will require hospital records and reports from your treatment providers. Other information may also be requested. You are responsible for the submission of this information. Your compensation analyst can provide direction on how to obtain the required documentation. The Board will pay for the completion of records and reports submitted in support of your claim. The Board will not pay for records or reports that have not been requested.
- If there is a criminal trial as a result of the incident, the Board will obtain documentation regarding the outcome on your behalf. If the matter is still before the courts, the Board may not be able to proceed with your hearing until after the trial is completed. This decision is made on a case by case basis.
- Once your file is complete, the Board will make arrangements for your hearing. You will receive a notice confirming the date on which this will occur. If your attendance is required at an oral hearing, your notice will provide you with a time and location. If you are unable to attend your scheduled hearing, please notify the Board immediately.
- You will receive a written decision by mail after your hearing. If you disagree with the decision, you should follow the instructions outlined in your decision. It is important that you respond quickly in these situations.
