How do I apply for compensation?
You must complete an authorized application form and submit it to the Board. There are two application forms, one for an injury and one for a death. You may either download an application form by clicking on the links below, or you may request an application form by contacting the Board by phone, mail or fax and one will be mailed to you.

You must have Adobe Reader 7.0 or a higher version installed on your computer in order to enter data in the application form, save it and print it. The latest version of Adobe Reader is available as a free download from www.adobe.com.

Here are some other important points to consider:
We must receive your application form within two years of the crime taking place. We can extend the two-year limitation period when it is warranted, but you will have to request such an extension and explain your reason(s) for the delay in applying. Click on When Do I File My Application? for more information.
Provide the Board with all of the requested information to ensure that your application is processed as quickly as possible. If you have a restraining order, probation order, victim impact statement, power of attorney, death certificate, medical or therapy records that support your injuries, invoices and/or original receipts, include these documents with your application. If you do not have these documents, do not wait to mail the application form to the Board. Gather the documents and then mail them to the Board once they become available. Remember to place your file number, which we will provide when you make your initial application, on any documents you send us later.
Remember to make copies of your application form and any supporting documents that you send to us for your records since these documents will not be returned to you.
All information collected by the Board is subject to the Freedom of Information and Protection of Privacy Act.
It is essential that you let us know whenever your address or phone number changes. If we are unable to reach you by phone or mail, your application may not be processed.
Mail your completed original application form to the Board with as much supporting documentation as possible to the address noted below. We do not accept faxed or photocopied application forms.
