How Do I Apply?
Forms are only available through the Criminal Injuries Compensation Board. You can request forms by phone, mail or fax.
The Board does not process faxed or photocopied forms or accept forms that are completed in pencil. The original forms that are sent to you must be completed in pen and returned to the Board. Please write legibly.
When you Need Someone to Apply on Your Behalf
If you are not physically able to complete the forms or if you are a minor, an applicant over the age of 18 may assist you. When completing the forms, please explain in writing the reasons you need an applicant.
Consent for Someone To Access Your File
You may give permission to another person to call the Board and ask about the status of your claim. You must provide the Board with written permission to speak with this person. This letter must be an original copy and must include your name, the date, your file number, your signature and the name of the person to whom you are giving authorization.
Giving a person authorization or consent to access your file does not give them the right to change information or make requests on your behalf. This is to protect your privacy and that of the material in your file.
Legal Representation
It is not necessary to have a lawyer, but you may choose to retain one. If you have a lawyer, the Board will only correspond with that lawyer. Any information about your file must be obtained through your lawyer. This also applies where you have contracted representation by an Agent or Paralegal.
The Board strongly discourages staff of other agencies from becoming the applicant on behalf of a person who is able to manage their own application unless the staff has been appointed as a guardian to the victim (e.g. Children’s Aid Worker or Trustee).
As the Board’s process requires an ongoing commitment, staff may not be able to dedicate the time required to follow up consistently to ensure the Board’s requirements are met. This can prove to be distressing to victims who rely on their advocacy and support.

All information collected by the Board is subject to the Freedom of Information and Protection of Privacy Act.
